At our practice, we are committed to providing care in a safe, respectful, and supportive environment for all patients, visitors, and staff. We kindly ask that everyone treat one another with courtesy, professionalism, and respect at all times.

To help maintain a positive experience for everyone, we enforce a zero-tolerance policy for abusive, aggressive, threatening, harassing, or disruptive behavior. This policy applies to all interactions, including in-person visits, phone conversations, emails, text messages, and other forms of communication.

Behavior that violates this policy may result in dismissal from the practice and/or refusal of future services.

We appreciate your understanding and cooperation as we work together to maintain a professional and respectful healthcare environment.


 
 
Referrals/Authorizations

​    Please be advised that patients are responsible for understanding their insurance benefits and requirements. If your insurance plan requires a referral or prior authorization from your Primary Care Provider (PCP), it is the patient’s responsibility to obtain and ensure approval is in place prior to services being rendered. Failure to obtain the required authorization may result in denial of coverage and patient responsibility for charges incurred.
 
 

Please be ready to  present a current  insurance card and picture ID.
